Premium Package $1480 (plus tax)
- Local pickup from the place of death

Payment
Payment in full is due prior to completing cremation rites. We accept cash, check, or credit card (Visa/Master Card, Discover, or American Express). We charge 3% fee for credit card payment. We accept insurance assignments for payment with a $200 processing fee.

- Illinois cremation permit
- Shipping of cremated remains via USPS
- Mileage for out of county deaths
Important Notes
- Stateline Cremations DOES NOT offer memorial services. We will provide a private viewing for an additional charge. We provide a cardboard temporary urn and will transfer cremated remains into an urn if needed.
- We will arrange a convenient time to obtain the necessary information. If desired we can arrange to meet in the family’s home, our office, or by telephone. The proper next-of-kin or Illinois Health Care Power of Attorney will need to be available to sign the required forms and authorizations at that time.
